Medicare guide

Help with Medicare costs may be available.

Explore programs that may reduce premiums, cost-sharing, or prescription expenses for eligible people.

Help depends on your circumstances

Two places to start with Medicare costs

Medicare costs

Medicare Savings Programs

May help with premiums and, depending on the program, deductibles or other cost-sharing.

Through Medi-Cal in California

Ask about the current application process.

Prescription costs

Extra Help

May reduce Part D premiums, deductibles and prescription copayments.

Also called LIS

Some people qualify automatically; others need to apply.

Eligibility and application rules differ. Care-at-home, dementia support and veterans programs are separate options covered below.

Medicare Savings Programs

Medicare Savings Programs help people with qualifying income and resources pay certain Medicare costs. Different program levels have different rules and may help with premiums, deductibles, or other cost-sharing.

In California, these programs are administered through Medi-Cal. Eligibility depends on the program and your circumstances. Ask about the current application process rather than assuming you do not qualify.

Extra Help with Part D

Extra Help, also called the Low-Income Subsidy or LIS, can reduce Part D prescription drug costs for eligible people. It may help with premiums, deductibles, and prescription copayments.

Some people qualify automatically through other benefits. Others need to apply. Have your current benefit notices available and ask what steps apply to you.

Care at home and community support

Medi-Cal waiver programs may offer services for eligible older adults or people with disabilities who need support beyond ordinary medical appointments. These programs have their own eligibility and availability rules.

Medicare does not generally pay for ongoing custodial long-term care. Separate support programs and long-term care planning can matter when a family needs help with daily activities.

Dementia care and caregiver support

The GUIDE model, Guiding an Improved Dementia Experience, is a Medicare dementia-care program delivered through participating providers. It can include care coordination and caregiver support for eligible people.

Ask a treating provider about participation and suitability. It is different from a Medigap policy or a general Medicare Advantage benefit.

Veterans and survivors

Aid and Attendance is a separate VA pension-related benefit for eligible veterans and survivors. A local VA office or accredited representative can explain that process.
