Put your new address into the coverage review.

  1. Share the move

    Tell your plan. Bring Julianne your new ZIP code and move date.

  2. Check your care

    Review doctors, hospitals, prescriptions and pharmacies at the new address.

  3. Coordinate the dates

    Understand the current plan end date, new start date and record updates.

Do not cancel your existing coverage before understanding what can continue, your enrollment rights and the new coverage start date.

A new ZIP code can make a difference

Medicare Advantage and stand-alone Part D plans are tied to service areas. A move to another city, state, or ZIP code can put you outside your existing plan's area or make other options available.

Before you move, tell your plan and ask Julianne to review the new address and move date. The timing of the notice can affect your Special Enrollment Period.

Check your care in the new location

A familiar insurer name does not guarantee the same provider network or drug coverage at your new address. Compare the actual plan available there.

  • Doctors, specialists, and hospitals near your new home.
  • Prescription coverage and a convenient in-network pharmacy.
  • The plan service area and new premiums or cost-sharing.
  • Your current plan end date and new coverage start date.
  • How to update Medicare, Social Security, and plan records.

If you have Original Medicare and Medigap

Original Medicare works with participating providers nationwide. Your Medigap policy may travel with you, but premiums, available options, and state-specific rules can differ. A separate Part D plan still needs a review when you move.

Do not cancel an existing policy just because you are relocating. Understand what can continue and which enrollment rights apply first.

A change in care setting also deserves a conversation

Moving into or out of a skilled nursing facility, long-term nursing home, or another institution can raise different coverage and enrollment questions. A hospital discharge, inpatient rehabilitation stay, or move to assisted living also calls for careful coordination.

These situations do not all create the same enrollment rights. Tell Julianne the type of setting, your dates, and current coverage. Ask the facility or discharge team about the services you need and how they will be covered.
